Fee Schedule

• Initial CONSULTATION/ new treatment initiation (up to 2 hours) – $450
• Continuation of treatment initiated in ER, hospital or rehab facility (up to 1 hour) – $225
• MAT initiation as referral from outpatient counselor with intake – $250
• Transfer of care/ continuation of long term treatment with greater than 3 months documented continuous treatment previously – $175
• Established patient follow up visits:
  • Up to 15 minutes – $125
  • 16 – 30 minutes – $250
  • 31 – 45 minutes – $375
  • 46 – 60 minutes – $500
  • Additional time – $125 for every 15 minutes or portion thereof
• Ambulatory Detox $500 – $700 per day (not all patients are suitable for ambulatory detox)

We do not accept/ bill ANY insurance or Medicaid

Sorry but we are unable to see any patients currently enrolled with any type of Medicare

Payment is due at the time of scheduling.

No shows, cancellations or rescheduling with less than 48 hours notice will forfeit the appointment fee
