Fee Schedule

Payment is due at the time your appointment is scheduled- appointments not paid within 24 hours of scheduling will not be held.

There is a strict 72-hour cancellation/ rescheduling policy. If you cancel or change your appointment less than 72 hours prior to the time of your appointment, you will forfeit the full fee for your appointment. By scheduling and paying for your appointment, you acknowledge and agree to this policy.

We do not participate with/ accept medical insurance

You will be provided with a “superbill”/ receipt that you may submit to your insurance for your insurance to reimburse you directly if you have coverage for out-of-network services.

We are sorry but at this time we are unable to see any patient who has ENROLLED with ANY Medicare plan or Medicare Supplement plan. By scheduling an appointment, you are attesting that you are NOT enrolled in ANY Medicare program/ plan.
